Sorry my practice is closed to new clients. I continue to support existing or previous clients who wish to contact me.  The following PDF forms may be downloaded by clicking on a link below (in the DOWNLOADS section) and then viewed with Adobe Acrobat. 

If we meet in person for a get acquainted time, please download the one page form by clicking on the link below: "Initial Get Acquainted Meeting,"  and then print and fill it out. Please bring it when you come.  Most of my "get-acquainted times" with clients  happen via phone or video.

Since COVID I  offer telehealth services for most of my clients, but do meet with some in person.-- For telehealth I use the doxy.me telehealth platform.  

If we decide to work together, you will need to fill out "New Client Packet 2024".  It provides the paperwork I will need from you for us to get started if we decide to do therapy. It includes an Informed Consent Form with more information on my background, how I do business, fees, and what your rights and responsibilities are. It also includes a two page Intake Form that you will need to fill out, as well as an Acknowledgement of Privacy Practices. You can ask me any questions you might have as you fill out the paperwork. If you intend to use insurance for one of the panels I'm on, please download and fill out "Redeeming Hearts Client Insurance Info" and bring it to our first session or have it all scanned in and ready on your own computer if we decide to use teleheatlh.

If you don't have time to read the packet and fill it out at home, we might be able to arrange for you to come 30 minutes early to a session and fill it out, or else we'll need to use our session time to review and fill out the forms. If you need me to email or snail-mail you the packet ahead of time, I can also do that. 

Likewise, my privacy practices (for Idaho) is found in the file: "Redeeming Hearts Notice of Privacy Practices".  I can also have a hardcopy of these forms printed out at my office if you request a hardcopy.
